CASE MANAGER

 DISTINGUISHING FEATURES OF THE POSITION:  This is an entry-level clinical position.  The work involves responsibility for counseling and offering guidance to a variety of clients in a clinical or residential setting.  Duties are performed as a part of an interdisciplinary team.  Work is performed under direct supervision by professional staff with leeway allowed for the exercise of independent judgment in individual cases.  Work is reviewed through clinical supervision and review of records.  The Case Manager performs related work as required.

 TYPICAL WORK ACTIVITIES:   (Illustrative only)

 ·      Participate as a member of the interdisciplinary team in    program planning, evaluation of client needs, and treatment   planning and implementation of direct services;

·        Participate in the planning and implementation of educational, vocational and training programs:  act as a consultant to client and makes referrals, as appropriate, to schools, social service agencies, health-related organizations, probation personnel, and other groups and agencies;

·        Counsel clients on basic addiction and recovery issues, living skills, the use of social and recreational outlets, and educational and vocational opportunities;

 Prepare and maintain required reports, case records, progress reports and similar documentation.

 REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
1)  Working knowledge of the principles, practices and techniques used in group and individual counseling. 
2)  Working knowledge of human behavior, socio-economic problems and development. 
3)  Ability to recognize psychiatric symptomatology and to participate in formulating effective treatment plans. 
4)  Ability to establish, maintain and terminate a counseling relationship.  
5)  Ability to establish and maintain effective working relationships with others. 
6)  Ability to communicate effectively, both orally and in writing. 
7)  Ability to maintain records and prepare reports. 
8)  Displays good judgment and tact.

RESIDENTIAL CASE MANAGER

 DISTINGUISHING FEATURES OF THE POSITION:  The Residential Case Manager carries a case load consisting of Community Residence clients as well as Supportive Living clients. COMMUNITY RESIDENCE
 PROGRAM AIDE

 DISTINGUISHING FEATURES OF THE POSITION:  The Community Residence Program Aide work involves the performance of important duties and liaison work with patients and public and private agencies so that the professional staff may devote more time to the complex and technical phases of their work.  The work is performed in accordance with policies and procedures established by the agency.  Program Staff will closely supervise work. The Community Residence Program Aide performs related work as required.

 TYPICAL WORK ACTIVITIES:  (Illustrative only)

Ÿ         Assist the professional staff in services related to treatment such as social training, recreation, follow-up studies, etc.

Ÿ         Conduct preliminary interviews with office visitors and may make field visits as assigned

Ÿ         Make necessary contacts to obtain referral data or other significant medical or psychiatric data

Ÿ         Take incoming calls and, if possible, secure necessary information for transmittal to professional staff

Ÿ         Explain agency policy and procedures in relation to personal data and substantiating documents

ADDICTIONS CRISIS CENTER
PROGRAM AIDE

 DISTINGUISHING FEATURES OF THE POSITION:  Program Aide work involves the performance of a variety of supportive duties that primarily assist professional staff in routine clinical and case management activities.  Additional duties are directly related to overseeing and monitoring clients as it relates to the agency schedule and client responsibilities.  Program Aides may work as a liaison with clients and community agencies so that the professional staff is able to devote more time to the complex and technical phases of their work.  The Case Manager on duty closely supervises the work of the Program Aides.

 TYPICAL WORK ACTIVITIES:  (Illustrative only)

Ÿ      Assist the Case Managers with needed clinical paperwork that includes psych-socials, treatment plans, treatment plan reviews.  Explain to clients extended care unit guidelines and responsibilities.

Take incoming calls, document the contents of conversations with other community agencies and transfer this information to the clinical staff.

Help the clinical staff by having clients sign required agency releases, monitor client phone calls to other agencies, and transport clients to various scheduled appointments and 12 step meetings.

Oversee, monitor, and assist clients in maintenance and housekeeping duties as well as all client meals.

Ÿ   Provide evening and weekend program coverage when needed.

Assist staff with intakes of new clients; check client belongings, monitor client smoke breaks, enforce agency policies and procedures, and insure client remains in the primary care unit at all times.

Follow instructions given by the clinical staff and complete any assigned agency task in a timely manner.

Ÿ         Fill in “Clinical Logbook” with any important client or clinical information that occurs during the course of assigned shift.

 REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: 
1)  Good oral and written communication skills;

3)  Ability to follow oral and written directions given by the professional staff;

LICENSED PRACTICAL NURSE

 DISTINGUISHING FEATURES OF THE POSITION:  The work involves responsibility for providing nursing care to the program patients to aid in prevention, treatment, and rehabilitation of alcoholism.  The incumbent performs a variety of nursing activities as part of a multi-disciplinary team, and provides health related training to other staff and carries a caseload of assigned clients.  Work is performed under supervision of the Program Director or Quality Healthcare Provider (QHP), with latitude allowed for the exercise of independent judgment in performing nursing duties.  Shift supervision may be a function of this position.  The Licensed Practical Nurse performs related work as required.

 TYPICAL WORK ACTIVITIES:  (Illustrative only)

·        Provide nursing services as a member of a multi-disciplinary treatment team;

·        Assess and identify patient nursing care needs. Prepare and carry out a nursing care plan, under the supervision of the Program Director or Quality Healthcare Provider (QHP);

·        Participate in the intake, screening, and diagnostic evaluation of patients and/or their family members;

·        Conduct assessments to identify factors in the social, psychological, and physical environment which may contribute to the disability;

·        May refer clients to other agencies for a variety of needed services;

·        Maintain contact with agencies such as schools, social services, d probation officers to understand and/or overcome clients’ medical and psychological problems;

·        Participate in and assist fellow team members in assessing, planning for, meeting and evaluation of patient and family member needs to aid in the formulation of a treatment plan;

·        Maintain medical records, medication sheets, and distribute medications as directed;

·        Attend courses and perform independent study and reading to further professional growth;

·        Compile and maintain a variety of records, reports, and statistical information.

 REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES: 
1)  Good working knowledge of the principles and practices of community health nursing. 
2)  Good knowledge of current trends and theories in the nursing care and treatment of patients with addiction or emotional problems. 
3)  Good knowledge of community organizations and health resources available for the addicted/alcoholic. 
4)  Working knowledge of abnormal signs and symptoms relating to physical illnesses.

DISTINGUISHING FEATURES OF THE POSITION:  The work involves planning, coordinating and managing the Supportive Housing Program for women in recovery who have children.  The incumbent will oversee the development and implementation of individual recovery plans for program participants, work with all involved parties toward reunification and stability of the mother-child unit, and offer guidance to clients by providing direct care, individual and group counseling.  Duties are performed under the direct supervision of the Supportive Living Coordinator with some leeway allowed for the exercise of independent judgment in individual cases.  Work is reviewed both directly and through conferences and analysis of records.  The family Empowerment Worker performs related work as required.

TYPICAL WORK ACTIVITIES:  (Illustrative only)

Maintain primary responsibility for the day-to-day operations of the Supportive Housing Program for women in recovery who have children.

Recruit and enroll eligible women for the program, including working with referral sources and social service departments.  Coordinate admissions, treatment and discharges.

Develop an individual recovery plan with each participant.  Educational, vocational, life skills, permanent housing and coping/problem solving goals with specific plans to meet these individualized goals will be included  Guidance and support in implementing the recovery plan will be provided as needed.

Work closely with all involved parties with regards to developing or supporting reunification and stability plans for each woman and her child(ren).  Provide parenting skills training, as well as education around human growth and development as needed.

Establish and maintain an individual case record for each participant, appropriately safeguarded as to confidentiality.

Meet at least weekly with new program participants to establish and implement their recovery plan and reunification/stability goals.  Meet bi-weekly thereafter to provide supportive counseling that fosters toward goal achievement.

Coordinate work with referral agencies, outpatient providers and child welfare providers to implement plans, especially around alcohol and chemical dependency counseling, vocational-educational training, housing readiness training, family counseling and parenting effectiveness training.

Coordinate regularly scheduled case conferences to review participant's progress on goals, overall performance within the program and progress toward achieving responsibility, self-determination and independence, free from alcohol and drugs.  Includes in all case conferences a focus on the mother-child unit, issues and goals to be addressed, and progress toward reunification or on-going stability.

Conduct regularly scheduled family counseling sessions as needed.

Lead weekly Supportive Housing Therapeutic Groups.

Meet weekly with assigned supervisor to discuss individual recovery plans, reunification and moth-child stability issues, programming issues and needs, occupancy rate, and planned admissions and discharges.  Provides supervisor monthly program reports and work collaboratively toward quality improvement.  Keep statistics as required by licensing body.

Prepare and maintain required reports, case records, and progress reports.

Plan and participate in recreation activities as needed.

Participate in the on-call rotation.

Participate as a member of the interdisciplinary team in program planning, evaluation of client needs, and service planning and implementation of direct services;

May staff and provide case management services to clients in non-traditional settings e.g. homeless shelters, crisis centers, social service agencies, etc; 

Participate in the planning and implementation of homeless shelter & housing, clothing, food services, etc.; act as a consultant and makes referrals, as appropriate, to shelters, social service agencies, health-related organizations, drug and alcohol treatment services, and other groups and agencies;
